{"id":6598,"date":"2022-01-17T15:06:56","date_gmt":"2022-01-17T14:06:56","guid":{"rendered":"https:\/\/blog.uchceu.es\/odontologia\/?p=6598"},"modified":"2022-01-17T15:07:02","modified_gmt":"2022-01-17T14:07:02","slug":"factores-de-virulencia-de-klebsiella-pneumoniae","status":"publish","type":"post","link":"https:\/\/blog.uchceu.es\/odontologia\/factores-de-virulencia-de-klebsiella-pneumoniae\/","title":{"rendered":"Factores de virulencia de Klebsiella pneumoniae"},"content":{"rendered":"\n<p><strong><span class=\"has-inline-color has-vivid-red-color\">\u00bf<\/span><\/strong><span class=\"has-inline-color has-vivid-red-color\"><strong>Qu\u00e9<\/strong> <strong>es <em>Klebsiella pneumoniae<\/em><\/strong><em><strong>?<\/strong><\/em><\/span><br>El g\u00e9nero Klebsiella fue descubierto en el siglo XIX por <strong>Edwin<\/strong> <strong>Klebs<\/strong>, un microbi\u00f3logo alem\u00e1n. <\/p>\n\n\n\n<div class=\"wp-block-columns is-layout-flex wp-container-core-columns-is-layout-9d6595d7 wp-block-columns-is-layout-flex\">\n<div class=\"wp-block-column is-layout-flow wp-block-column-is-layout-flow\" style=\"flex-basis:100%\">\n<p><em>K. pne<\/em>umoniae es un bacilo Gram negativo de la familia <em>Enterobacteriaceae<\/em>. Esta bacteria est\u00e1 presente de forma <strong>comensal en la microbiota<\/strong> <strong>oral<\/strong> y digestiva as\u00ed como en las v\u00edas respiratorias superiores de humanos y animales. <\/p>\n\n\n\n<p>Los \u00faltimos estudios  demuestran que juega un papel muy importante en el desarrollo de enfermedades infecciosas cuando se encuentra en el intestino y la faringe, incluyendo: neumon\u00eda, infecci\u00f3n del tracto urinario, meningitis; especialmente en personas d\u00e9biles y\/o inmunocomprometidas.<\/p>\n\n\n\n<h2 class=\"alignfull wp-block-heading\"><span class=\"has-inline-color has-vivid-red-color\"><strong><em>K.pneumoniae<\/em> &#8211; microbiota oral-enfermedades infecciosas<\/strong><\/span><\/h2>\n\n\n\n<p><\/p>\n<\/div>\n<\/div>\n\n\n\n<div class=\"wp-block-columns is-layout-flex wp-container-core-columns-is-layout-9d6595d7 wp-block-columns-is-layout-flex\">\n<div class=\"wp-block-column is-layout-flow wp-block-column-is-layout-flow\" style=\"flex-basis:100%\">\n<div class=\"wp-block-group\"><div class=\"wp-block-group__inner-container is-layout-flow wp-block-group-is-layout-flow\">\n<div class=\"wp-block-columns is-layout-flex wp-container-core-columns-is-layout-9d6595d7 wp-block-columns-is-layout-flex\">\n<div class=\"wp-block-column is-layout-flow wp-block-column-is-layout-flow\" style=\"flex-basis:100%\">\n<p>Sin embargo, ha evolucionado hacia una <em>K. pneumoniae<\/em> hipervirulenta que puede enfermar incluso a personas sanas. Es una de las p<strong>rincipales causas de las infecciones hospitalarias.<\/strong> Adem\u00e1s de los abscesos piog\u00e9nicos hep\u00e1ticos,<em> K. pneumoniae<\/em> hipervirulenta se diferencia de <em>K.pneumoniae<\/em> cl\u00e1sica por su capacidad de propagaci\u00f3n a sitios distantes (met\u00e1stasis), m\u00e1s com\u00fanmente el ojo, los pulmones y el sistema nervioso central (<em>Choby et al., 2020<\/em>). Esta patogenicidad se debe a su capacidad de adaptaci\u00f3n y de desarrollo de nuevos factores de virulencia as\u00ed como de multirresistencia a antibi\u00f3ticos como la ampicilina, las tetraciclinas y los beta-lact\u00e1micos (<em>Moradigaravand et al., 2017). En consecuencia, K. pneumoniae hipervirulenta se ha convertido en un verdadero problema de Salud P\u00fablica<\/em>.<\/p>\n\n\n\n<figure class=\"wp-block-image size-full\"><img loading=\"lazy\" decoding=\"async\" width=\"569\" height=\"479\" src=\"https:\/\/blog.uchceu.es\/odontologia\/wp-content\/uploads\/sites\/7\/2022\/01\/image.png\" alt=\"\" class=\"wp-image-6601\" srcset=\"https:\/\/blog.uchceu.es\/odontologia\/wp-content\/uploads\/sites\/7\/2022\/01\/image.png 569w, https:\/\/blog.uchceu.es\/odontologia\/wp-content\/uploads\/sites\/7\/2022\/01\/image-315x265.png 315w, https:\/\/blog.uchceu.es\/odontologia\/wp-content\/uploads\/sites\/7\/2022\/01\/image-150x126.png 150w, https:\/\/blog.uchceu.es\/odontologia\/wp-content\/uploads\/sites\/7\/2022\/01\/image-300x253.png 300w\" sizes=\"auto, (max-width: 569px) 100vw, 569px\" \/><\/figure>\n<\/div>\n<\/div>\n<\/div><\/div>\n<\/div>\n<\/div>\n\n\n\n<h2 class=\"wp-block-heading\">\u00abSeg\u00fan la OMS, <em>K. pneumoniae<\/em> forma parte de la lista de los 12&#8243;agentes pat\u00f3genos prioritarios\u00bb resistentes a los antibi\u00f3ticos\u00bb<\/h2>\n\n\n\n<p><strong><span class=\"has-inline-color has-vivid-red-color\">FACTORES DE VIRULENCIA<\/span><\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Los factores de virulencia de <em>K.pneumoniae son:<\/em><br>1) <strong>C\u00e1psula<\/strong><br>Esta bacteria produce una capsula viscosa de polisac\u00e1ridos complejos. Esta capsula permite impedir el enlace con fagocitos y entonces evitar fagocitosis. <br>2) <strong>Fimbrias<\/strong><br>Las fimbrias participan en la formaci\u00f3n de colonias bacterianas intracelulares. Son factores importantes para la formaci\u00f3n de biopel\u00edculas. <em>(Clegg &amp; Murphy, 2016).<\/em><\/li><\/ul>\n\n\n\n<div class=\"wp-block-image\"><figure class=\"alignright size-large is-resized\"><img loading=\"lazy\" decoding=\"async\" src=\"https:\/\/blog.uchceu.es\/odontologia\/wp-content\/uploads\/sites\/7\/2022\/01\/image-3-650x311.png\" alt=\"\" class=\"wp-image-6620\" width=\"564\" height=\"256\" \/><\/figure><\/div>\n\n\n\n<ul class=\"wp-block-list\"><li>3) <strong>Ureasa:<\/strong> Esta enzima contribuye a la formaci\u00f3n y crecimiento de bacterias, que crecen en el tracto urinario y que forman c\u00e1lculos de infecci\u00f3n. La hidrolisis de la urea provocara una precipitaci\u00f3n de sales inorg\u00e1nicas (e incremento de pH) que conducen a la incrustaci\u00f3n, principalmente en los cat\u00e9teres a un flujo de orina deteriorado y formaci\u00f3n biopel\u00edculas que pueden inhibir el tratamiento de antibi\u00f3ticos.<\/li><li>4) <strong>Formaci\u00f3n de biofilm<\/strong><br>La formaci\u00f3n de biofilm por <em>K.pneumoniae<\/em> promueve la resistencia a la muerte del hu\u00e9sped y antimicrobianos. Adem\u00e1s, la comunidad de biopel\u00edculas es m\u00e1s resistente a la acci\u00f3n de muchos antibi\u00f3ticos. La matriz del biofilm,  matriz densa de prote\u00ednas, de polisac\u00e1ridos y de ADN, previene la difusi\u00f3n eficaz de antibi\u00f3ticos. Las biopel\u00edculas formadas por <em>Klebsiella pneumoniae<\/em>, seg\u00fan los estudios mas actuales, presentan resistencia  a beta-lact\u00e1micos o ciprofloxacino. <\/li><li>5) <strong>Resistencia a los antibi\u00f3ticos<\/strong>: en este apartado destacamos dos mecanismos de acci\u00f3n.<br>La producci\u00f3n de enzimas betalactamasas (contra cefalosporinas y penicilinas) y las enzimas carbapen\u00e9micos (contra todos los beta-lact\u00e1micos, incluyendo carbapen\u00e9micos, cefalosporinas, penicilinas y monobact\u00e1micos).<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC7503635\/\">https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC7503635\/<\/a> <\/li><li><em>Klebsiella pneumoniae<\/em> es multirresistente a una gran variedad de antibi\u00f3ticos por lo que el tratamiento es muy complicado.<\/li><\/ul>\n\n\n\n<div class=\"wp-block-image\"><figure class=\"alignright size-full\"><img loading=\"lazy\" decoding=\"async\" width=\"523\" height=\"348\" src=\"https:\/\/blog.uchceu.es\/odontologia\/wp-content\/uploads\/sites\/7\/2022\/01\/image-1.png\" alt=\"\" class=\"wp-image-6613\" srcset=\"https:\/\/blog.uchceu.es\/odontologia\/wp-content\/uploads\/sites\/7\/2022\/01\/image-1.png 523w, https:\/\/blog.uchceu.es\/odontologia\/wp-content\/uploads\/sites\/7\/2022\/01\/image-1-315x210.png 315w, https:\/\/blog.uchceu.es\/odontologia\/wp-content\/uploads\/sites\/7\/2022\/01\/image-1-150x100.png 150w, https:\/\/blog.uchceu.es\/odontologia\/wp-content\/uploads\/sites\/7\/2022\/01\/image-1-300x200.png 300w\" sizes=\"auto, (max-width: 523px) 100vw, 523px\" \/><\/figure><\/div>\n\n\n\n<h2 class=\"wp-block-heading\">\u00abLos dos factores de virulencia m\u00e1s importantes son la capacidad para desarrollar la capsula y resistencia a muchos antibi\u00f3ticos\u00bb<\/h2>\n\n\n\n<p><\/p>\n\n\n\n<p><\/p>\n\n\n\n<p><\/p>\n\n\n\n<p><\/p>\n\n\n\n<p><\/p>\n\n\n\n<p><\/p>\n\n\n\n<p><\/p>\n\n\n\n<p>Para concluir, podemos decir que <em>Klebsiella pneumoniae<\/em> es un bacteria con un papel muy importante en la aparici\u00f3n de enfermedades infecciosas cuando se encuentra en partes del cuerpo como el intestino y la faringe. Puede producir graves infecciones nosocomiales como: neumon\u00eda, infecci\u00f3n del tracto urinario, meningitis, etc. Tiene m\u00faltiples factores de virulencia como la ureasa (que provocar\u00e1 una precipitaci\u00f3n de sales inorg\u00e1nicas), sider\u00f3foros (las bacterias ser\u00e1n m\u00e1s invasivas, porque un exceso de hierro en los tejidos del hu\u00e9sped favorece la patogenicidad bacteriana).<\/p>\n\n\n\n<p> Adem\u00e1s, es muy resistente a los antibi\u00f3ticos gracias a la producci\u00f3n de enzimas betalactamasas, enzimas carbapen\u00e9micos y tambi\u00e9n su formaci\u00f3n de biofilm.
