Puede ser causada por bacterias, hongos, virus u otros microorganismos. La endocarditis bacteriana se refiere espec\u00edficamente a la infecci\u00f3n por bacterias.\u00a0 Es una de las enfermedades m\u00e1s graves que existe y tiene una alta tasa de mortalidad. Poco frecuente, en Espa\u00f1a, es una enfermedad con una incidencia estimada de entre 3,1 y 3,7 episodios cada 100.000 habitantes y a\u00f1o, que es m\u00e1xima en personas de edad avanzada (m\u00e1s frecuente en adultos con edad media de 50 a\u00f1os).<\/p>\n<p>Se produce por la llegada de microorganismos circulantes al torrente sangu\u00edneo y a la<br \/>\nsuperficie de las v\u00e1lvulas an\u00f3malas. Los microorganismos que m\u00e1s frecuentemente causan la infecci\u00f3n son <strong><em>Streptococcus del grupo viridans \u00abSGV\u00bb (<\/em><\/strong><i>S.mutans, <\/i><i>S.oralis, <\/i><i>S.milleri, <\/i><i>S.salivarius) <\/i>(Imagen 1). Heine et al 2019.<\/figcaption><\/figure>\n<h3 style=\"text-align: center\">\u00bfC\u00f3mo puede ocurrir?<\/h3>\n<p>Durante de una extracci\u00f3n dentaria, alrededor de un tercio de las personas presentan <em>Streptocococcus<\/em> del grupo <em>viridans en<\/em> la sangre. Normalmente estas bacterias son r\u00e1pidamente eliminadas en la circulaci\u00f3n. Estructura qu\u00edmica de Clindamicina.<\/figcaption><\/figure>\n<p>Anteriormente se consideraba que para prevenir la endocarditis era imprescindible tomar antibi\u00f3ticos antes de someterse a determinadas intervenciones odontol\u00f3gicas o quir\u00fargicas para minimizar el riesgo de que una infecci\u00f3n se desarrolle. Esta teor\u00eda no es avalada por todas las asociaciones de Odontol\u00f3gos e intentan minimizar el uso o abuso de la toma de antibi\u00f3ticos.<\/p>\n<p>El antibi\u00f3tico m\u00e1s utilizado es <strong>\u00abAmoxicilina\u00bb (Imagen 2),<\/strong> penicilina semi-sint\u00e9tica que forma parte de la familia de las \u00df-lact\u00e1micos, que puede prescribirse en combinaci\u00f3n con<strong> \u00e1cido clavul\u00e1nico.\u00a0<\/strong> El uso de amoxicilina en el sector de la odontolog\u00eda tiene como ventaja que es un antibi\u00f3tico de amplio espectro, es decir, tiene acci\u00f3n frente bacterias gram positivas y gram negativas. Concretamente,\u00a0 tiene actividad frente a diferentes <strong>periodontopat\u00f3genos<\/strong> como <em>Actinobacillus actinomycetemcomitans (cocobacilos gram negativos), <\/em><em>Peptostreptococcus spp. (cocos gram positivos en cadena), Fusobacterium spp.(bacterias filamentosas gram negativas) y Estreptococos orales (Glenny et al. 2013)<\/em>.<\/p>\n<blockquote>\n<h3>\u00abAmoxicilina-Clindamicina\u00bb<\/h3>\n<\/blockquote>\n<p>En el caso de que el paciente presente alguna alergia al antibi\u00f3tico anterior, puede tomar clindamicina (Imagen 3), aunque los \u00faltimos estudios ya identifican <em><span style=\"text-align: center\">Streptococcus <\/span><\/em><span style=\"color: #222222;font-family: Verdana, Geneva, sans-serif;font-size: 15px\"><em>viridans<\/em> resistentes a la clindamicina (Adriana et al. 2019).<\/span><\/p>\n<h3 style=\"text-align: center\">\u00abEn Espa\u00f1a, la resistencia a amoxicilina<br \/>\nasociada con \u00e1cido clavul\u00e1nico por sobreconsumo es una preocupaci\u00f3n importante\u00bb.<\/h3>\n<h3 style=\"text-align: center\">\u00bfQu\u00e9 factor de virulencia presentan los <em>SGV<\/em>?<\/h3>\n<p>Un factor de virulencia de <em>SGV<\/em> es la presencia de <strong>\u00e1cido lipoteicoico<\/strong> (Imagen 4) que favorece la adherencia del microorganismo a las v\u00e1lvulas card\u00edacas y la producci\u00f3n de muchos polisac\u00e1ridos extracelulares que da adherencia entre ellos, lo que es un dispositivo importante en la endocarditis.
